Entry Forms Available At the Library

CONTEST RULES:
1. Read four books or listen to four audio books between June 1 and July 31
2. For each book read, write the title, author, and a brief description on the
entry form.Check the box next to your favorite title.
3. Turn in your completed entry form at the Reference Desk to earn a
library card wallet with key ring.
Your name will also be entered to win one of the following prizes:
a $250 Apple gift certificate, a $100 Lake Forest Chamber of Commerce gift certificate,
a $50.00 gift certificate from a local restaurant (Southgate, Mia Francesca,The Grille on Laurel )
or one of 5 $50 gift certificates to Williams-Sonoma.
Prizes must be claimed by September 1 or another winner will be drawn.
4. You must be a Lake Forest resident 18 years or older to participate.
5. One entry for each resident cardholder.
